Plan-Preferred Provider Network (P.P.N.) This
plan will reduce your massage therapy expenses
on all products and services. There are no annual
or lifetime maximums, no claim forms to file,
no exclusions, and there is no pre-authorization

to receive the scheduled member benefits. Make
